The following forms are for graduate students only.
Registration
Graduate Course Preauthorization Request
- Complete this form when you need “instructor permission” or “departmental” preauthorization to register for an SOJC graduate course.
- Submit this form each term you’re approved to register for JCOM 604, JCOM 609, JCOM 503, and JCOM 603 credits.
- We will be able to clear you to register after we receive your completed Internship Tracking Form, Terminal Project Proposal Approval Form, Thesis Proposal Approval Form or Dissertation Proposal Approval Form.

Internship Tracking Form
Complete this form when an SOJC faculty member has agreed to serve as the instructor for your JCOM 604 Internship credits.
Terminal Project Proposal Approval Form
Complete this form before requesting to enroll in JCOM 609 Terminal Project credits.
Thesis Proposal Approval Form
Complete this form before requesting to enroll in JCOM 503 Thesis credits.
Dissertation Proposal Approval Form
- Complete this form when requesting dissertation approval.
- Upon the Graduate Programs Office receiving this form, the Advancement to Candidacy process begins (it can take anywhere from a few weeks to a few months for the Division of Graduate Studies to finalize).
Petitions
Add/Swap After Deadline Petition
Use this form to petition to add or swap a course. Swap refers to changing sections of the same course or adjusting the level in a language, math or science sequence. (Fees apply: $25 to add course after the add deadline; $100 during initial registration after week 4 of the term; $50 to add course for a prior term.)
Late Drop/Withdrawal After Deadline Petition
Use this Petition to Drop Without a Recorded W, or Withdraw with a Recorded W, after the drop deadline of the term. (Approved petitions are subject to a fee of $25-$50. If the petition is for initial registration for the term, a $100 late registration fee may apply after the 4th week of the term.)
SOJC Professional Master’s Required Course Waiver Peition
Please be aware of the Required Course Waiver Petition Deadlines for each term; Fall term: September 1; Winter term: December 1; Spring term: February 15. (You are still required to fulfill your program’s minimum number of credits to graduate as indicated in the graduate program handbook.)
General Petition
This petition may be used to request exceptions to Division of Graduate Studies regulations only, not program specific exceptions. The petition must be endorsed by your graduate advisor and by the head of your school or department before it can be submitted to the Division of Graduate Studies for action.
Petition for Reinstatement
If you have missed one or more terms without filing for on-leave status, use this form to request reinstatement to your graduate program.
Refund Petition
This petition is for an exception to established university policy and requires supporting documentation for petitioning.
Removal of Incomplete Petition
The SOJC policy requires that graduate students not have incompletes in more than two classes.
Communication and Media Studies Student Required Course Waiver Petition Form
Both CMS Master’s and PhD students can complete this petition to request a waiver from a required course.
Communication and Media Studies PhD Course Transfer Petition
PhD students can complete this petition to request a previous course be transferred in to count towards their degree. A syllabus for the transfer course and sample work you completed for the course must also be submitted.
CMS Graduate Students: Complete the appropriate form and work with your advisors to begin the review and potential approval process.
Scheduling a Defense
Thesis/Project/Dissertation Defense Scheduling Form
Complete this form when scheduling your thesis/project/dissertation defense. (This form is required for the announcement to the SOJC community.)
